A confirmation email will be sent to you upon submission, and our treasurer will be
notified to process your request. Fields marked * are required.
- Use an email address you will have access to after graduation — your confirmation and any follow-up will be sent there.
- Enter the address where you would like the check mailed.
- Your check will be made payable to both you and the academic institution or program you will be attending.
- Please allow up to 2 weeks for processing and delivery.
